The EPSG code your X / Y values are in. The layer itself is always stored and published in WGS 84, so everything else keeps working unchanged.
Select a column with sequential IDs (1, 2, 3…) or leave as "Auto-generate".
Altitude must use the same height reference as your point cloud. Heading is degrees clockwise from north; leave it derived from the path unless your camera does not face the direction of travel.
A rotation matrix also gives pitch and roll, which no other source can provide. Do not use Omega/Phi/Kappa here: on a near-horizontal camera those angles do not carry the heading.
Nine columns, in the order M11 M21 M31 M12 M22 M32 M13 M23 M33 that photogrammetric exports list them in.
